Spain has started the operational rollout of a common system for applying notified medicine prices in community pharmacy. The Notified Price System (Sistema de Precios Notificados, PRENO) provides the infrastructure needed to distinguish dispensing financed by the National Health System (Sistema Nacional de Salud, SNS) from that supplied outside public funding and to reconcile the resulting price difference.

Spanish law already allows a financed medicine to carry a lower SNS financing price and a higher notified price for private dispensing. It also requires pharmacies, distributors and manufacturers to provide the information needed for the associated financial adjustments. The practical difficulty has been applying that distinction to individual dispensing from common pharmacy stock.

The Spanish Medicines Verification System (Sistema Español de Verificación de Medicamentos, SEVeM) and the General Council of Official Colleges of Pharmacists (Consejo General de Colegios Oficiales de Farmacéuticos, CGCOF) developed PRENO as a common reconciliation mechanism. Specialist reporting in July said the system would use SEVeM infrastructure to identify dispensing outside SNS funding and support the corresponding financial settlements between pharmacies and upstream suppliers.

Professional pharmacy bodies moved into implementation during August. The Official College of Pharmacists of Málaga issued dispensing and invoicing conditions on 17 August. The Official College of Pharmacists of Gipuzkoa subsequently announced the ‘launch’ of PRENO on 24 August.

Spain has used notified prices for more than a decade, including a narrower reconciliation arrangement introduced in 2013 for medicines removed from general reimbursement but retained for selected funded indications. PRENO extends the practical infrastructure for differentiated pricing across community pharmacy. It should make wider use of separate SNS and private prices technically feasible as Spain develops its new financing and pricing framework.
